Insurance
As of January 1, 2023 we will no longer be participating/accepting any insurance plans.
- Self-Pay Only
Patients who have a PPO plan may be eligible for out-of-network benefits which will reimburse a percentage of their payment in accordance with the provisions of their plan. We will be happy to provide you with a document commonly referred to as a "Superbill" for you to submit to your insurance company for reimbursement. You will need to contact your insurance company for information on their policies/procedures for getting reimbursed.
